Prize Acceptance Agreement

1. Marketing and Promotion: I hereby permit the use of my name, image, and likeness for the promotional purposes of USA Archery (Promoter) for the USA Archery Sweepstakes in any media for an unlimited period of time without further notification, remuneration or compensation for the purpose of promoting the USA Archery Sweepstakes, USA Archery and to promote any products manufactured, distributed and/or supplied by the USA Archery that I have won in the USA Archery Sweepstakes (including third party promotional, marketing and publicity purposes).
2. Prize Distribution: I am providing consent for USA Archery to disclose my personal contact information to third parties for the purposes of conducting the USA Archery Sweepstakes and for purposes of delivering the Prize.
3. Taxes: I understand that if I accept a prize valued at $600 or more, I will personally be responsible for paying local, state and federal income tax on my winnings; and I will be issued a 1099 tax form for the value of the prize I won.

Participant Consent

By signing below, I agree that I have read this Agreement completely and carefully, understand the terms and conditions I am agreeing too and intend for my signature to serve as confirmation of my complete and unconditional acceptance of this Agreement.
